Effect of lifestyle, medication and ethnicity on cardiometabolic risk in the year following the first episode of psychosis: prospective cohort study

Background
The first episode of psychosis is a critical period in the emergence of cardiometabolic risk.
AimsWe set out to explore the influence of individual and lifestyle factors on cardiometabolic outcomes in early psychosis.
MethodThis was a prospective cohort study of 293 UK adults presenting with first-episode psychosis investigating the influence of sociodemographics, lifestyle (physical activity, sedentary behaviour, nutrition, smoking, alcohol, substance use) and medication on cardiometabolic outcomes over the following 12 months.
ResultsRates of obesity and glucose dysregulation rose from 17.8% and 12%, respectively, at baseline to 23.7% and 23.7% at 1 year. Little change was seen over time in the 76.8% tobacco smoking rate or the quarter who were sedentary for over 10 h daily. We found no association between lifestyle at baseline or type of antipsychotic medication prescribed with either baseline or 1-year cardiometabolic outcomes. Median haemoglobin A1c (HbA1c) rose by 3.3 mmol/mol in participants from Black and minority ethnic (BME) groups, with little change observed in their White counterparts. At 12 months, one-third of those with BME heritage exceeded the threshold for prediabetes (HbA1c >39 mmol/mol).
ConclusionsUnhealthy lifestyle choices are prevalent in early psychosis and cardiometabolic risk worsens over the next year, creating an important window for prevention. We found no evidence, however, that preventative strategies should be preferentially directed based on lifestyle habits. Further work is needed to determine whether clinical strategies should allow for differential patterns of emergence of cardiometabolic risk in people of different ethnicities.

Aphids and scale insects on threatened trees: co-extinction is a minor threat

Co-extinction is the extinction of a species following the extinction of another species that it used as a resource, such as a food plant in the case of insect herbivores. The magnitude of the global co-extinction threat to two herbivorous insect taxa (aphids and scale insects) was estimated by compiling a list of species in these groups that are dependent on globally threatened trees. Eleven species of aphid (0.69%) and thirteen scale insects (1.15%) have a threatened tree as their sole host. This measure is comparable to recent estimates for insect herbivores, but far less than the published overall estimates of extinction risk for invertebrates, and highlights the dependence of insect herbivores on a wide range of habitat features.

Background Cognitive-behavioural treatments have previously been explored in the treatment of chronic psychotic problems, but recently, the effectiveness of these treatments has been investigated with regard to recent onset and acute psychosis.
Method The literature relating to cognitive-behavioural treatments in psychosis is explored and the application of the approach to recent onset psychosis is described in detail.
Results There appears to be a growing body of evidence that the advances made in the treatment of people with chronic treatment resistant psychosis can be similarly applied to people with recent onset and acute psychosis.
Conclusions Cognitive-behavioural treatments are feasible with recent onset psychotic patients although further evaluation of their effectiveness is necessary.

Summary
There are a number of reasons for the increasing interest in outcomes of health care interventions over the last few years. Foremost is the realization that in all health care systems, resources are limited, and should be directed towards those interventions that are of proven effectiveness, and producing outcomes that are valued by patients. As will be discussed below, deciding upon the outcome to be achieved is a necessary prerequisite for determining whether any treatment is effective.
The next stimulus to outcomes research is the realization that there are large variations in practice, not only throughout the world, but within the same health care system, and indeed in neighbouring cities. For example, Wennberg's comparison between Boston and New Haven, two university cities on the East coast of the United States, showed that in Boston people had twice the chance of having a carotid endarterectomy compared to New Haven, but only half the chance of having coronary bypass surgery (Wennberg et al. 1987). These differences were apparent even when corrections were made for the age and sex distribution and other variables between the two local communities. Another striking example is that rates of hysterectomy correlate more closely with the number of gynaecologists per head of population than they do with the number of women within a population. Such variations indicate that much of what doctors do is a matter of practice style, is imprecise, and is not related to procedures of proven effectiveness. Belatedly, the precision that has been brought to bear in biomedical research is now being brought to health services research.
